Some things I've learned the hard way: Kill all unnecessary background Windows processes. CFG BufferPools tweak is well worth it with a modern GPU, no reason not to do HIGHMEMFIX, AffinityMask fix redundant. Not really worth bothering with DX10 Preview. If you're having stuttering problems and your framerate is obviously not to blame, remember to run a test flight twice after every tweak you try. The game engine often has to use CPU resources to recompile the shader caches after a tweak, which it will do the first time it encounters a new situation. In my experience, as long as your settings and expectations are appropriate to your hardware specs you'll often find the stuttering reduced or even gone the second time you run a test scenario.
If you've done a lot of tweaking it might be worth flushing the shaders caches (delete the contents of User/Yourname/Appdata/Local/Microsoft/FSX/Shaders) and letting the game rebuild them from scratch over a series of test flights.
